We can add .zel programs to the start-up suit manually using the appload dialog box.

Is it possible to do it programatically using autolisp code?

In ZWCAD installation directory, there is a file called "AppAutoLoad.app"(if not, you can create one by yourself, using Windows Notepad), it is used to load specific files into start-up suit. You can just write down your file path and file name in this file, then it will be added to start-up suite automatically.
Such as: C:'Documents and Settings'test.lsp

So if you want to use lisp code to do it, you can refer to below:
(setq file (open "AppAutoLoad.app" "a"))
(princ "<path>'<filename>" file)
(princ "'n" file)
